Medical Forms

All participants at off-site activities must fill out BOTH of the following forms and bring them to the event.

• Annual Medical Form 680-001 Parts A & B (2019 printing)
• COVID-19 Pre-Screening Checklist 680-058 LHC (1/26/21 rev)

At the end of the event you can choose either:

• take the medical forms home with you
• leave a copy in the green binder, where they can be kept for up to one year
